Pharmaceutical reps, primary care providers, therapists, school counselors, community service providers, parents, and/or others involved in the care and/or treatment of an established patient that I follow may request a telephone/office meeting using this form.

Please follow these instructions:

1. First, click on the following link to be sent to my patient portal site: https://www.valant.io/myio/RachelGFolger

2. Log in to my patient portal using the following credentials:

           Username: Meeting_Request

           Password: 12345ABCd#

3. Click on the "Request an Appointment" button.

4. For appointment type choose "Meeting with Collateral Provider".

5. Choose your preferred date range and click "search". All open appointments for those dates will be listed. Choose one by clicking on the green "Request" button.

***6. Once you choose your preferred appointment date and time, return to this page and complete the information below so that the provider will know who they are meeting and the reason for the meeting. If the form below is not completed the appointment request will be canceled.
