New Patient - Portal Instructions
Becoming a new patient or the parent/guardian of a child with a new provider can be very frustrating. Every practice has their own way of doing things; their own workflow. Each practice may also use a different electronic health record program or EHR. In the mental health field there are many programs that you may have never heard of. In my practice I use a program called Valant. Valant was designed by a psychiatrist, frustrated with the shortcomings of EHR software built for primary care and other medical disciplines. I feel that this software best meets the documentation needs of behavioral health providers and their practice, as well the partient.
Below you will find detailed instructions on how to create your patient portal, access your portal, update your demographic information (address, phone number, email, etc.), and complete the initial intake documents prior to your first appointment. The process to become a new patient begins with a patient referral, this referral can either be sent by a parent/guardian, another provider such as a primary care physician, therapist, crisis evaluator, or anyone with legal custody of the patient. If the patient is 18 years or older, the patient can refer themself.

The welcome screen should appear as the following:
On this first screen please enter the name of the person requesting the referral, indicating whether you are requesting a referral for yourself or someone else. Then click the next button. Please fill out as much information as you can (not just the minimum required by the program).
Once the referral is received by the provider, the provider will review the referral to see if the patient meets the criteria for the practice, or if they need another provider who may be more appropriate considering the age, problems, and/or the patient's type of insurance.
If the prospective patient referral is accepted by the practice, the practice will create a patient account and then request that the parent/guardian or patient (if 18 or older) activate the patient portal account. The patient portal activation request may be sent by the practice as either an email or a text message. *For most of the intake paperwork/portal setup, the use of a computer is recommended. Examples are shown below; the text invitation first, and email invitation below it:
The email or text message invites you to download the MYIO app for your mobile device. The MYIO app allows you to check to confirm appointments or to securely message your provider with any concerns or medication refill requests.
The first step to creating your portal is to click on the link. The link should take you to a screen that looks like the following:
From this point enter the access code provided to you in either your text message or in your email, then click next. After clicking "next" you will be asked to setup the account.
Your user name and password can be anything that is easy for you to remember. Enter the requested information for the person that will be accessing the portal. This would either be the parent/guardian or the patient if they are 18 or older. After you enter your account information and complete the security questions you will land on the page below:
From this point click on the "start" button to begin your patient portal onboarding (onboarding is just a fancy way of saying getting started or beginning the process of becoming a patient and setting up your patient portal). After you click on the start button you will be brought to the next screen.
For this screen, just click on the "next" button to begin and to proceed to the next page which should pertain to the patient, not the parent or guardian. You may omit the social security number if you're concerned about providing that information.
Personal details 2 is also about the patient.
The following screen is about email addresses for the parent/guardian or the patient if they are 18 or older.
The portal email address is usually the same as the home email address. However, you are able to use another email address if desired. Once you click on the "next" button you will be sent a verification code, either by text or email depending on how the initial request was sent to you. Just enter the verification code to proceed.
When your email address or phone number is verified, click the "next" button. The contact information should be for the patient.
Contact information 2 includes the preferred method of receiving patient appointment reminders. All questions and refill requests for medication should be made through the patient portal. Please be sure include insurance information. The address and phone number for the insurance company is not necessary. *Please be sure to email the practice copies of the patient's insurance card. If the patient has commercial health insurance insurance (not MassHealth), the practice will also need the name, address, and date of birth for the parent providing the health insurance.
After you finish entering the insurance information and click the "next" button you must accept the Updated Terms of Use". If you decline to accept, the patient referral may be rescinded.
After you click on the "Accept" button you will be brought to your patient portal homepage.
